The Birthday Club is a fun way to celebrate!

 

Send me an e-mail via the Contact page including the following information:

Fill out:

Your Name

Your phone number

Your contact email

Then in the Comment area:

Type in "Birthday Club" and

Your Date of Birth

 

That's it. In your birthday month, I will send you an email that you will print off and bring to your appointment (within 15 days of your actual birthday). You will then be allowed to pick a gift from the Gift Basket! Follow the instructions in the email (takes precedence over what is written here.)
